Health Benefits – Medical/Prescription, Dental, & Vision:

Your medical, dental and vision coverage remains in effect until September 30, 2020. All other benefits will terminate effective your termination of employment. With the continuation of your medical coverage your care and your provider network remain accessible to you. If you have questions regarding your benefits and claims, doctors and/or help with symptom please reference the numbers on the back of your medical insurance card. Remember that the 24-hour access center and NowClinic remain available to you for symptoms and support, or call your provider directly.

UPDATED INFORMATION IN REGARDS TO COBRA
Continuation of Benefits (COBRA):
Effective 10/01/2020 continuation of health coverage shall be available to you and/or your covered dependents (Qualified Beneficiaries). The Qualified Beneficiary will be required to pay premiums for any period of COBRA coverage equal to 102% of the applicable premium (see chart below). United Healthcare Benefit Services (UHCBS) mailed notifications to your home address on September 23, 2020 for continuation of benefits (COBRA). If you wish to elect COBRA coverage, you must respond within sixty days of the date you receive notification. Premium payments for COBRA coverage are paid directly to UHCBS.
The cost of COBRA may be subject to future increases during the period it remains in effect. For more detailed information, refer to the Summary Plan Description (SPD) available in your Human Resources office or call UHCBS directly about this process at (866) 747-0048 or via email COBRAKYoperations@uhc.com or visit their website at https://www.uhcservices.com . The monthly premiums (as of January 1st, 2020 – rates are subject to change each year) are as follows:

Are There Other Coverage Options Besides COBRA?

Yes. Instead of enrolling in COBRA continuation coverage, there may be other coverage options for you and your family through the Health Insurance Marketplace, Medicaid, or other group health plan coverage options (such as a spouse’s plan) through what is called a “special enrollment period”. Some of these options may cost less than COBRA continuation coverage. You can learn more about many of these options at www.healthcare.gov or The Silver State Health Exchange www.nevadahealthlink.com (800) 547-2927 Monday to Friday 9am – 5pm, Saturday – Sunday 10 am – 2pm.

Retirement Plan (401k):

If you are currently enrolled in the 401(k) program and have an account balance that is equal to or less than $5,000, then a distribution must be processed by completing a Distribution Form. You may roll over your funds to another Retirement Plan, IRA, or have the funds paid directly to you. Penalties may apply. See Special Tax Notice.

If your account balance exceeds $5,000, your funds can remain active with the Station Casinos LLC & Affiliates 401(k) Retirement Plan. You are 100% vested in your contributions; however, any company match you received will be subject to a vesting schedule.

COMPANY PAID PLANS: OPTIONS:
Company-paid Short Term Disability This coverage is not portable, which means that you cannot continue coverage after you leave the company. Your existing coverage will end on your last day of employment.
Company-paid Life Insurance Company paid Life Insurance can be converted into a whole life individual policy. Please contact the Team Member Benefits Center at (866) 254-8758 within 31 days of your termination date for rates and information.

 

VOLUNTARY PLANS: OPTIONS:
Voluntary Life Insurance Voluntary Life Insurance coverage is portable, which means that you can take it with you. For rates and information, please contact the Team Member Benefits Center at (866) 254-8758 within 31 days of your termination date.
Voluntary Short Term Disability This coverage is not portable, which means that you cannot continue coverage after you leave the
company. Your existing coverage will end on your last day of employment.
Long Term Disability This coverage is not portable, which means that you cannot continue coverage after you leave the company. Your existing coverage will end on your last day of employment.
MetLife Accident/Critical Illness/Hospital Indemnity Insurance This coverage is not portable, which means that you cannot continue coverage after you leave the company. Your existing coverage will end on your last day of employment.
Met Law Coverage ends upon date of termination. You may contact Met Law at (888) 821-6400 for coverage continuation information.
MetLife Home/Auto Insurance This coverage is portable, which means that you can take it with you. You will be responsible for contacting Met Life at (800) 438-6381 regarding rates and the payment of your premiums.
Flexible Spending Account (FSA) Health and Limited Health FSA coverage is portable, which means that you can take it with you. For more information, please contact the Team Member Benefits Center at (866) 254-8758 within 31 days of your termination date.
